You can easily download all the forms on this page in a single .ZIP file which contains everything all at once. This gives you all the .TXT versions of the contracts so that you can alter them to suit your own productions. It also includes the .PDF versions of charts and call and log sheets. Backfocus Plane
This backfocus chart can be printed on a plain 8.5"x11" sheet of paper
and used to check focus on your camera before you shoot. Simply put the chart about 10 feet from the
camera, open iris, zoom in, and focus. Adjust back
focus on lens until chart is sharp (no morie-pattern) in middle of chart. Zoom out to
wide. If chart remains sharp, back focus is correct.
Call Sheet
This is a customized version of a "traditional" Hollywood call sheet. It is a .PDF that you can type onto and print. You can set it up with the title of your film, your production company address, the call times, the dates, everything you could need for a simple call sheet.
CD & DVD Logos
Need to put your own custom CD together? Created a DVD that you want an official logo for to put on the packaging or the label? Use these logos. Note that these logos are NOT to be used on commercial releases but ONLY for private use. The DVD logo, for example, is trademarked and must only be put on discs which meet certain specifications.

DV Log Sheet
Log sheets are used to record scenes and takes for motion pictures in order
to sort out the coverage that was aquired during shooting. This simple log sheet is designed for digital films. It allows you to
track tapes, scenes and timecodes. The sheet should be filled out ON THE SET
as the scenes are being shot. In order to minimize confusion during the
editing process, we recommed that you use only ONE log sheet per videotape - do not
attempt to record multiple tapes or multiple scenes on a single logsheet as it
will just make the editing process that much more complicated. Don't forget to record "room tone" for EACH of your scenes either. Finally, you may wonder, "Why is the SCENE and the SHEET NUMBER printed so BIG on the log sheet?" Simple. When you have 60 log sheets spread out all over a tackboard (or, more likely, "over the floor" if you are an independent editor) it makes it easier to read them at a glance.
EIA Resolution Chart 1956
The EIA chart is a video camera, standard resolution test pattern. Print the image so that it fills the page. Do not crop
any of it. Affix the printout to a wall. Frame it inside your viewfinder so the triangles
at the edge just touch the outside of your frame. The convergence where
the lines in the wedges fade into one another, is the number of "lines of resolution" of your camera.
There are much more details instruction on using charts like this for
professional use, but this test is adequate for prosumer camera equipment
sans a vectorscope.
Group Release (Unpaid)
You may have a large group of extras on set and you don't want to have dozens upon dozens of people all signing individual release forms. This form contains several lines so that multiple people can sign a single release.

Intern Agreement
This agreement is used for outlining things between interns and the production. This can entail bringing people on as Production Assistants or into more well-defined roles. This form is for unpaid intern positions.

everything in writing regarding the dates and times that you will be there and all

of the project.

Movie Ratings
Have an independent film that you want to apply a rating to? The ratings bestowed by The Classification and Rating Administration are registered trademarks - you can't just arbitrarily give your own movie a PG rating and slap a "PG" logo on the film. But these ratings are designed in the same style and are for independent, unrated movies, to give your posters and promotional items more flair and originality.
Prop Loan Agreement
Sometimes, you will need to borrow or loan a prop from someone for a project. If you purchase something outright, obviously you don't need a contract. But, if you are going to be returning something that you are using, you will want to be clear on what you are borrowing and when you will return it to the rightful owner and how much the item is worth in case it gets damaged during use.
